Extensive surgery for peritoneal tuberculosis, an ongoing diagnostic challenge in resource limited setup

Background: Peritoneal Tuberculosis is an abdominal form of that affects the peritoneal cavity and enclosed organs. Clinical presentation this rare extrapulmonary tuberculosis resembles advanced ovarian cancer may results in unnecessary extensive surgery especially resource limited setting. Case presentation: A case a 36 years old prisoner who presented to us with gradual onset distension for one-month, mild pain noticeable progressive weight loss. Physical examination revealed she was underweight, afebrile had healed sub umbilical median incision scar on glossily distended abdomen positive fluid thrill shifting dullness. She HIV negative, anemic marked elevation Cancer Antigen 125 marker. Abdominal ultrasound scan showed ascites multiple cysts originating from left iliac fossa pelvic CT showing cyst, 3 litres diffuse carcinomatosis. Advanced suspected intra-operatively 2.5 liters straw colored ascitic found, bowels omentum were covered intestinal nodules inclusion occupying cavity. Histopathological analysis sampled tissues Tuberculosis. Conclusion: shares similarities should be ruled out woman before proceeding surgery. For proper differentiation two, histopathological tissue through frozen section biopsy preferred approach setup where laparoscopic or guided not feasible.
